How to Start Home Care for a Loved One

Support your aging loved one’s dignity, happiness, and safety at home with Grace Hands Home Care. We simplify the process of starting home care, providing guidance and reassurance at every step.

Easy, Personalized Home Care Onboarding

The first step is scheduling a free, in-person consultation with one of our care coordinators (also known as a “care manager”). This meeting is a great opportunity for you and your family to ask questions and receive personalized guidance. It also helps the care coordinator get to know you and your loved one’s specific needs.

During the consultation, your coordinator will discuss key aspects of care, such as physical needs, scheduling, and any special requests, ensuring that the care plan is comprehensive and customized.

Common topics covered during the consultation include:

  • Physical Needs
Your coordinator will determine the level of care required, such as assistance with bathing, grooming, dressing, meals, mobility, incontinence care, and more.

  • Emotional Needs
Elderly companion care can include social interaction, conversation, activities, hobbies, and even assistance with appointments, ensuring your loved one stays engaged and connected.

  • Homemaking Assistance
If your loved one needs help around the house, we can provide assistance with meal preparation, grocery shopping, laundry, linens, and light housekeeping.

  • Schedules
We will create a care schedule that aligns with your loved one’s daily routine, including sleep, eating, and medication reminders.

  • Logistics
We’ll review all the practical details, such as access to the home, necessary documentation, communication with caregivers, and how to stay in touch with your local office.

Customized Care Planning

After the initial consultation, we’ll work with you to create a personalized care plan, finalize the schedule, and match your loved one with the caregiver best suited to meet their needs.
