Live-In Care

Does your loved one need in-home care? Grace Hands Home Care offers Certified Nursing Assistants (CNAs), Geriatric Nursing Assistants (GNAs), and Home Health Aides (HHAs). Our caregivers are compassionate, skilled, and certified to assist with daily activities, providing your loved one with personalized care to ensure a comfortable, fulfilling life.

Care That Goes Above and Beyond

Activities of Daily Living (ADLs) include essential tasks such as eating, bathing, personal hygiene, dressing, toileting, continence, and general mobility. While many senior care providers assist with these tasks, our aides go further by offering additional services such as medication reminders, meal preparation, light housekeeping, laundry, and exercise assistance. We also provide transportation services, ensuring your loved one can attend doctor’s appointments, go shopping, or visit the pharmacy.

Our clients frequently express appreciation for these extra services because we genuinely care for their loved ones as if they were part of our own family. This level of dedication ensures your loved one enjoys the best possible quality of life.

Personalized Care Plans

At Grace Hands Home Care, every client receives a custom-tailored care plan. Registered Nurses conduct periodic visits to monitor the effectiveness of the care plan, assess the client’s health and well-being, and ensure their environment is safe and supportive. These regular check-ins allow us to adjust the care plan as needed and provide peace of mind for families.

Steps to Receive In-Home Care

Complimentary Evaluation
Your loved one will receive a free evaluation from one of our Registered Nurses, who will assess their goals, unique needs, and the level of care required. To streamline the process and reduce stress, our Director of Nursing, Director of Care, and Company Representative will visit together for a comprehensive assessment.

Creation of a Custom Care Plan
Based on the evaluation, our nurse will develop a personalized care plan focused on your loved one’s physical and psychosocial needs. Social interaction is an essential aspect of our care, so we make sure to incorporate meaningful engagement into the plan.

Caregiver Matching
We carefully match your loved one with a caregiver who meets their needs and complements their personality. The relationship between caregiver and client is vital, and our thoughtful matching process ensures a strong bond and a positive, nurturing environment.

A Typical Day in In-Home Care

A typical day with our caregiver begins by assisting your loved one out of bed, followed by help with bathing, dressing, and using the restroom. The caregiver will then prepare nutritious meals and provide assistance with eating. To support physical health, the caregiver will encourage and assist with exercise, such as a walk around the neighborhood or simple therapeutic activities.

Our aides also focus on your loved one’s social and emotional well-being, offering companionship and creating a stimulating environment. They build strong relationships with clients, fostering a sense of trust and friendship that contributes to your loved one’s overall health and happiness.

At Grace Hands Home Care, we are committed to ensuring your loved one is safe, healthy, and thriving in their home environment.
